Access to quality primary care is fundamental to achieving equitable health outcomes for all individuals, regardless of their socioeconomic background or geographic location. However, in many underserved communities, barriers to accessing primary care persist, contributing to significant health disparities. These disparities result in unequal health outcomes, higher rates of chronic illnesses and reduced life expectancy among vulnerable populations.
